NOTE:

NOTE:
When submitting your form in STEP #3, please send it directly to the
CPPEA address on the form.

Submitting this form is Step #1 in taking back a portion of your Union Dues that SEIU has taken out of your monthly paycheck automatically. The amount that you will save is about 2% of your dues, so you will still be paying about 98% of your dues to SEIU.

VERY IMPORTANT:
This Petition should be Filled in, then Printed out, Signed and Mailed.
You can print it out and fill it in by hand if you can’t fill it in online.

The only thing you lose by filing this form is the right to vote on internal SEIU elections such as for SEIU Officers, etc.

After submitting this form:
  • SEIU must still represent you in Bargaining and Grievances.
  • SEIU must treat you the same as a full dues-paying member.
  • You will become a “Fair Share” fee payer.

You will still qualify to vote on Decertifying SEIU as your Union even after filing this “SEIU Membership Cancellation Form” .

After submitting the SEIU Membership Cancellation form above, go to” Step #2 – Non-Germane Form” to save up to 45% of your SEIU Union Dues.
